What is an advance directive? An advance directive is a written legal document that allows you to name a person to make medical decisions for you if you were unable and or document what type of medical care you would want or not want.
-
Who is an advance directive for?An advance directive is for everyone 18 and older with the ability to make decisions and communicate. Advance directives are done in advance of illness or injury that prevent us from being able to communicate or make decisions- and since we can't predict when that might happen, we should take this step now. Your advance directive is for you, but it also has the potential to impact your loved ones. This document is for you, and is a gift to those you love.

Where do I find blank advance directives? Each state determines their requirements for that state's advance directives. Most states have several versions that meet the state requirements, so know there isn't "one right form", there may be many right versions to choose from. You can find one for your state by asking your primary healthcare provider or searching "Advance Directive (name of your state)" or by heading to this page that has links for every state: ( https://www.everplans.com/articles/state-by-state-advance-directive-forms)

When should I complete an advance directive?Since we can't predict when the ability to communicate or make decisions might be lost, it is recommended that you complete your advance directive as soon as you are able.

How do I know if the advance directives at home or 1:1 discussion is right for me?It really depends on your learning style and preference. Advance directives at home gives you everything you need to complete your own advance directive, and talk with anyone about this. Upon purchase a recorded training will be emailed to you, and you will have access to this for 30 days. This is a great option for people who want time to digest this information, and be able to go at their own pace. For most people this will be all they need. If you complete the advance directives at home, and have additional questions, email us at support@buoyadvancedirectives.com and we'll give you a coupon code for a discounted 30 min 1:1 session to review your questions. Some people would rather just talk it out from the start, for those people the 1:1 sessions are a great option. The sessions will be held on Zoom, and will address your specific questions/concerns. For either option, please have a blank advance directive for the state you reside in.

I want someone I care about to complete their advance directive, how can I encourage them? The best way to encourage anyone to complete their advance directive is to start by completing your own. Then you can tell them about your experience, and share that because you care about them, you want to make sure their wishes are documented so they can be honored. This conversation is much easier when you can speak from experience.

How long will it take me to complete my advance directive? The time it takes to complete your advance directive can vary greatly depending on individual approach, and the version of advance directive you choose. Having said that, once you've thought and talked about this a bit, the actual paperwork can be completed in as little as 10 minutes. Signing requirements vary from state to state as well, so there may be some additional time required to bring together witnesses, named medical POAs, or a notary as required.
